COVID-19 Prevention Related – Protection of Personal Data Announcement

Due to COVID-19 epidemic, visitors must cooperate to fill in the “Health Declaration Form”. For people who have been to or contacted with restricted area defined by CDC in the last 14 days, must comply with epidemic prevention actions.

Your personal data collected by TSMC via this “COVID-19 Health Declaration Form” contains your personal identification information, physical condition, contact information, data concerning your social activities, and other data that is necessary for us to take anti-epidemic measures. Your personal data will not be used for any purpose other than the aforementioned epidemic prevention purposes (“Collection Purpose”).

Your personal data will only be used in TSMC Museum of Innovation, and within the necessary and reasonable scope for the Collection Purpose until the Collection Purpose is no longer applicable.

Pursuant to Article 3 of the Taiwan Personal Data Protection Act, you may have the following rights regarding your relevant personal data: the right to request access to the collected personal data, request a photocopy of the collected personal data, request supplement or rectification of inaccuracies in the collected personal data, request us to cease the personal data collection, processing or use, or delete the collected personal data.
